The IOTN has two parts:

Dental Health Component (DHC): Scored 1–5, this measures the functional and structural impact of the malocclusion. It considers factors such as overjet, overbite, crossbite, crowding, missing teeth, and impactions.

  • Grade 1–2: No or mild need for treatment. Not normally funded.
  • Grade 3: Moderate need. Borderline — some cases are funded, others are not.
  • Grade 4–5: Severe need. Routinely funded.

Aesthetic Component (AC): Scored 1–10 using reference photographs, this assesses the visibility of the dental irregularity. High aesthetic scores can support funding in borderline cases.

What Scotland, Wales, and Northern Ireland offer
